Perhaps, with romance, you s – x y bru-ute,

There’s SOMETHING SPECIAL – that factors in,

Something beyond?! and – linked with-sin?

For, maybe, sin’s-NOT a nasty thing,

Perhaps something-SACRED – does-our-romance bring!

Yeah! OK! Perhaps – I’m just-“a goner,”

And, perhaps – we-are-but-thieves – and-there-is-NO-honor,

Or, maybe, my-“mummy-wife” with “special-traits,”

Is more than you see!

The World waits,

To-see-if-this-couple- is-like- ”the-one-down-the-street,”

Who-have-been-together-forever – having-forgotten how-they-did-meet!

The Key to Managing Chronic Eczema

Your skin is an amazing organ: It has a very capable barrier function, a good blood supply to call in nutrients, and the ability to repair itself. It has its own local immune surveillance system, a support system with collagen, elastic fibers, etc. (like a scaffolding on a construction site), and its own system of nerves.

But sometimes that brilliant design is disrupted, and problems, like chronic eczema, emerge. One of the most common causes of chronic eczema is genetics – some people have genes that cause alterations in the skin’s barrier properties and immune functions, which makes them more likely to get eczema, grow certain bacteria, and have ongoing problems. Chronic eczema is also caused by the environmental irritations – friction, chemicals in soaps and skincare products, cold or dry weather, etc. And, believe it or not, eczema is often made chronic by the very the medications, especially creams and antibiotics, that doctors give patients to try to treat the problem.

Treatments Sometime Backfire

While the oft-recommended steroid creams do have an anti-inflammatory function, if used more than 2-3 weeks, they start to inhibit collagen production, impair the skin’s natural immune functions, and decrease cell turnover (which, over time, leads to thinner and more susceptible skin). And antibiotics, while sometimes necessary when eczema leads to infection, alter and impair the natural “biome” on the skin, robbing it of all the good microorganisms (which are protective).

So, these treatments meant to help your eczema can actually make your skin thinner, more susceptible to friction, more susceptible to bacteria, and less able to mount its own immune response and natural anti-inflammatory activity.

Focus on Restoring Skin Health Instead

So rather than get stuck in the endless cycle of eczema and infections where relapse occurs as soon as treatment is stopped, what you really need to do is restore the health of your skin. When eczema has been present for a long time, building up the skin’s natural defenses will take a while. It is a slow process, usually taking 2-4 months – but it’s worth it. Here are some ways you can start:

First rule of protecting “broken” skin: moisturize!! Literally, 20-30 times a day at first. Of course, what you moisturize with is important. Try to find a “body or hand cream” that is quite thick (not thin), convenient, not too expensive, as chemical-free as possible, and really feels good on your skin. Some of our patients do fine with Vanicream (not the lotion) or CeraVe cream, some Shea butter, or things like the L’Occitane hand cream with Shea butter (but has fragrance), or olive oil creams. (see Dr. Irwin’s glossary of skincare ingredients).
Remember that even natural skin creams can have irritating things in them. Some of the natural creams have plant extracts that can cause problems, as people with eczema often have allergies to plants and pollens. Try some different ones to find one you love and tolerate well (no stinging, burning, itching etc.).
If you have hand eczema, every time your hands touch water, you must re-moisturize.
Absolutely no commercial soaps ever! Not even once a month. You cannot use the soaps in hotels, airplanes, airports, movie theaters, offices etc. Carry a small bottle of your own cleanser with you or just use water.
Learn to read the basics of labels. Many prescription creams for eczema have awful chemicals in them. Avoid propylene glycol, synthetic fragrances, preservatives (as much as possible), and lanolin/wool wax alcohol for starters. Creams with very short lists are better than creams with long lists. Avoid “natural” creams with a lot of plant extracts in them.
If you’ve been using a steroid cream longer than about 3 weeks, you can’t just “cold turkey” off it. You have to gradually wean your skin off its addiction so the skin’ natural processes can gradually take over. This takes about 2 months. If you’ve been using the steroid cream daily, start by diluting it to half strength with cream and still using it daily. In 1 -2 weeks, if your skin is doing well, go down to every other day for 2 weeks, then 2 times a week for two weeks, then once a week for two weeks and then off. Wean slowly – you get the idea!
To get away from antibiotics, consider using treatments with antibacterial properties instead. Try using a silver gel daily; if it burns or stings or is too drying, stop using it. You can also try Hibiclens (a gentle antiseptic liquid) available at the pharmacy. You’ll gradually need less oral antibiotics as your skin gets healthier.
Take a probiotic daily (a good quality one is usually refrigerated).
If you suddenly have a flare-up, try to identify what caused it. Inadvertently used a commercial soap? Ate a triggering food? Feeling emotional or stressed? Also, consider allergy testing with an allergist and allergy “patch” testing with a dermatologist.
If you have a bad flare-up, try to use the steroid cream for only 5-7 days and then start the taper again. The goal is to be off them in 3-4 weeks.
Your eczema may be caused by foods. Until your eczema is healed, try to avoid acidic foods like all citrus, including lemons, tomatoes etc. Get a list of acidic foods online. You may need to experiment to determine the specific culprit, but go off everything first for 2 weeks and then slowly add foods back one at a time, about 2 weeks apart.
Eat a healthy diet with lots of veggies, whole grains, and clean proteins. Avoid sugar, processed anything, white flour, too much bread, pastry, pasta, alcohol (moderation is fine), too much salt. Skin is an organ, and it can’t be healthy if we aren’t.

Is Sunscreen Dangerous? A Dermatologist Weighs In

It’s becoming more and more difficult to figure out what to believe about anything – even something as basic as sunscreen. This is partly because companies, and some individuals, have figured out how to use the power of suggestion and incomplete (or false) information, to direct our thought patterns in favor of, or against, certain products.

Some news stories that popped up this summer might lead you to believe that there’s a troubling new safety concern with sunscreen. The stories were centered around a single study published in a lesser-known journal (Chemosphere). Journalists took partial truths from the study and amplified them into more partial truths. The echo effect! And voila – unfounded hysteria!

What the study really said was that one ingredient (avobenzone) in some sunscreens could, when exposed to chlorine and light in a swimming pool, convert into a small amount of toxic chemical (which would then be diluted in a very large swimming pool). Note that this was not tested in an actual swimming pool. It was tested in a lab under conditions – NOT at all like the real world.

So, how should you respond?

If you are swimming in a chlorinated pool, avoid the ingredient avobenzone for now, until we have better science.
Don’t drink quarts of pool water. Swallowing a little bit will not be an issue.
Don’t spend a lot of time licking your skin in a chlorinated pool. Not something most of us do anyway.
What you shouldn’t do is abandon sunscreens altogether. Why?

They help protect you from the deadliest form of skin cancer – melanoma
They help protect from the other types of skin cancer, basal cell, and squamous cell carcinomas
They keep your skin youthful, unwrinkled, and help prevent sagging
They prevent brown spots and blotchiness
They prevent precancerous spots (actinic keratosis)
All of the above is according to verified science, from a multitude of research.

The bottom line is, safety is an issue for all of us. Is it safe to cross the street or drive your car? Not completely. But we accept these risks. Is it safe to eat food? Yes, but not all food (sugar, soft drinks, pesticide-treated veggies, fried foods – probably not so much). The degree of risk we take all depends on the way we think about risks in our environment for ourselves, and our families.

Are You Getting ‘Sneaky’ Sun Damage?

Many of us think that only direct exposure to sun is dangerous for our skin – we assume that “natural light” is fine.

Think again! UVA, which causes brown spots, uneven pigment, wrinkles, and skin cancers, is present in all natural light. It goes through clouds, through the windows in your home and office, and through your car glass. So, it’s present around you pretty much all day.

And just a little bit of UVA, all day, every day, really adds up.

To keep your skin beautiful and healthy, avoid UVA as much as possible, especially if your skin tone is lighter. The lighter your skin tone, the less natural protection you have against sun damage. UVA is sneaky – it tends to NOT cause sunburns, and instead, just damages skin slowly.

Most sunscreens now block UVB well (the SPF number is referring to UVB protection). But sunscreens do NOT tell you how much UVA they block. Your sunscreen may be labeled “broad spectrum,” but essentially that means nothing.

Only zinc (in higher concentrations) effectively blocks UVB and UVA well. The concentration you need depends on how much you are outdoors and in your car and what you do outside. For example, a good concentration for just a little outdoor exposure – like a run in the early morning – in the spring/summer might be 5-10% zinc.

If you are cycling outdoors for hours and sweating, or if you’re working outdoors, aim for a concentration of more like 10-20% zinc – and be sure to reapply when needed.

And keep in mind, UVA is the strongest two months on either side of summer solstice day. So, those in the US (and elsewhere in the northern hemisphere) need to be especially vigilant in May, June, July and August (those in the southern hemisphere need to mind the months around December solstice).

Anti-Aging Tips for Your Hands

You probably have anti-aging products and strategies for your face – but what about your hands? They age, too! Like our faces, they are susceptible to sun damage, environmental insults like soaps and hand sanitizers, and just everyday “wear and tear.” And since we need our hands to do almost everything, they tend to look older than our faces.

Here are a few of the challenges that our hands face (and ways you can help):

Problem: Hands get washed countless times during the day long, which has a drying effect on the skin — which, of course, contributes to wrinkling.

What to do?

Try to apply moisturizer to your hands after every exposure to water. Use something fairly thick – a cream not a lotion.
Also, use products similar to the ones you use on your face on your hands: an antioxidant in the morning under sunscreen and a gentle retinol or tretinoin at night.
Problem: Hands get a lot of sun, which destroys the already-thin skin. It’s all that sun exposure, not aging, that causes most of your brown spots. Sun exposure also reduces skin thickness, collagen, and elasticity.

What to do?

Reapply sunscreen as much as you can.
Hands get a surprising amount of sun while driving because UVA goes through car window glass, so use driving gloves to cover the backs of the hands. I’m a fan of the Bloxsun gloves – they are easy to get on and off, can be washed, leave your fingertips free and have a slight grip for the wheel.
For damage already done, consider corrective treatments like IPL/photorejuvenation or Fraxel Dual. IPL (intense pulsed light) is the first choice because it doesn’t break the skin at all, and you can use your hands right afterwards for normal activities. The Fraxel Dual requires special care for your hands for a few days but also targets brown spots. It builds collagen more than the IPL.
Problem: Hands lose fat as we age. And when it comes to our hands, losing fat is not a good thing: the fat in your hands acts as cushioning around your veins, making them look less prominent. So, less fat means more visible veins.

What to do?

Try using Juvederm or Restylane for volume to plump the skin on the back of your hands and camouflage the veins.
With great basic care and a preventive approach, there is no need for hands to age faster than your face. Investing even a little time in your skin now will really pay off in the future.

Is Your Skincare Right for Your Age?

Lotions, Botox, Dysport, Fillers, Lasers, Skin Tightening…it’s great to have so many skincare options, but how do you know when it’s time to start using them? At what age should you start thinking of adding one (or more) of these to your routine? And, is it ever too late to try to fix something?

Here are some good general guidelines:

In Your 20’s:

It is NEVER too early to start sunscreening your face, neck, chest and hands every morning. Start in your teens or 20s if you can. That 30 seconds you invest once in the morning will save your skin years of aging.
Establish (and stick to) a solid basic skincare routine – a good antioxidant under your sunscreen in the morning followed by a good moisturizer. At night, use a vitamin A cream like retinol (non-prescription) or Renova/tretinoin (prescription).
Don’t “fix” problems you don’t have. If you are 25, and have no frown lines, starting Botox/Dysport just wastes your time and money. Start only when you see problems developing; and who knows, you may get lucky and never have need.
In Your 30’s:

By their mid-30’s, most women and men are starting to show signs of frown lines, crow’s feet, or upper lip lines. This is usually a good time to start preventive treatments with small doses of Botox/Dysport 2-3 times a year– but only if you actually have these problems.
Almost everyone can benefit from photorejuvenation/IPL starting in their 30’s. It clears up small brown spots, dilated blood vessels, and generates a little collagen which improves skin texture.
Over 40:

Fillers can help with early signs of volume loss (sagging), which affects most women/men by the age of 40-50. Starting with a syringe or two of hyaluronic acid filler 1-2 twice a year retards the sagging. Everyone is very individual on this, so consult your doctor for an individualized plan for correction of existing sagging and then maintenance.
Laser treatments need to be customized at this age. You might be someone who can benefit from skin tightening like Thermage/Ultherapy, etc. or you might need more intense laser skin improvement like the Fraxel Dual or fractionated carbon dioxide laser – it really varies by the individual, so you will need a consult.
Over 60? Your skin can still be noticeably and safely improved with these technologies, it just takes a little more time, more treatments and more expense.
And, some good advice for all ages:

Don’t believe everything you hear from marketers or friends. Do your own web research (on reputable sites) and talk to your doctor, but also take into account your own skin and its reactions. We are all different, so what works for others might not work for you.
Don’t start more than one new skincare product at a time.
Remember it often takes 1-3 months to see a beneficial effect of a new skincare product.
